Alex Kummer, Administrator

Hello, my name is Alex Kummer and I am the Administrator at Baldwin Care Center, Inc. I am originally from Rock Falls, Wisconsin where I grew up on a dairy farm and also raised pigs for over 10 years. I then received my Bachelor’s Degree in Health Care Administration from the UW-Eau Claire in May of 2022. I have worked in the health care field since 2019 working in assisted livings, nursing homes, and most recently at Sacred Heart and St. Joseph’s Hospital in Eau Claire and Chippewa Falls. I currently live in Chippewa Falls with my boyfriend as well as my dog (River) and cat (Poppy). Outside of work, I enjoy reading, baking, fishing, kayaking, watching movies, and playing with my pets.

My responsibilities as the Administrator include directing and managing the overall operation of Baldwin Care Center, Inc., including the Baldwin Care Center, Birch Haven and the Birchwood Apartments. I am excited to take on this new role and work with the team here to provide quality care and services to all residents and their families.

Jenell Neidermyer, Birchwood Apartment Manager

My name is Jenell Neidermyer. I am from the Roberts area and currently live south of Woodville with my family. I have three grown daughters and raise beef with my husband. My hobbies include reading, being outside, walking, and spending time with family. I graduated from North Central University with a degree in education and a minor degree in missions. While in college, I started helping people maintain independent living by working for an in home health care company.

Baldwin Care Center, Inc. first hired me in 2016 as a resident assistant at Birch Haven and then, later as manager of Birchwood Apartments. My role as manager is to ensure that each tenant feels comfortable and independent as possible in their home. The staff and I work hard to provide a secure and affirming environment with activities, outings, and social interactions that promote physical, psychological, and emotional well being.

It has been and still is my pleasure to work at Birchwood Apartments with these amazing people!

Jessica Bark, Director of Nursing Services

My name is Jessica Bark.  I have been in healthcare for over 24 years.  I have my Bachelor’s Degree in Nursing and have done many facets in the nursing field, including long term care, medical-surgical floor, obstetrics, intensive care units, emergency room, behavior health/psychiatric, surgery, endoscopy, interventional pain management, and same day surgery, to name a few. This, however, is my first management job!

I am married to Richard Bark, who is retired from the Marine Corps and currently an over the road truck driver, and we have three children.  Shannon, who is 26, lives in Menomonie with her dog and works in Woodbury at a group home for Autistic young men.  Eric, who is 21, goes to college at University of Wisconsin, River Falls.  There he plays football and is majoring in Environmental Science with minors in Hydrogeology and Conservation.  He will graduate in December 2023.  Then, there is Christopher, who is 18.  He was our last one to graduate in the year 2022.  He will be going into Welding and is currently looking for someone to work for.

I’m excited to be back in Baldwin, as this is where I started my healthcare career 24 years ago as a CNA!  It is an honor to be back in long term care with those who we learn so much from!  Thank you for letting me be here for your loved ones!

Kathryn Jacobson, Social Services Director

Hello, my name is Kathryn Jacobson. I recently joined the Baldwin Care Center team in July of 2018 as the Director of Social Services. I have a Bachelor's Degree in social work from the St. Catherine's University in St. Paul, MN and I have worked as a social worker in a skilled nursing facility setting for almost 30 years. I live in Hammond, with my husband and we enjoy sports, cooking, and spending time with family and friends.

My role is to advocate for the residents by providing choices, encouraging self determination and promoting person centered goals. I wish to work with the residents and their families to ensure positive communication while building mutual trust and respect.

Some of my responsibilities as the Director of Social Service include assisting with the admission process by providing education to staff and residents regarding psychosocial well being, mood and lifestyle changes/difficulties that may happen during the adjustment process. I also assist with discharge planning by ensuring residents and their families are provided with safe, successful discharge options including a discharge meeting, referrals to community resources and education prior to discharge. I am available to give a tour, discuss the specific needs of a resident prior to their stay at Baldwin Care Center. I look forward to working with the residents and families that call Baldwin Care Center their home.

Danielle Lausted, MS RD CD, Registered Dietician

Hello, my name is Danielle Lausted. I have been the Dining Services Manager and Registered Dietitian at the Baldwin Care Center since October of 2014. I received my Bachelor of Science Degree in Dietetics in 2001 as well as my Master of Science Degree in Food and Nutritional Sciences at the University of Wisconsin-Stout in 2003. I have worked in the nutrition field since I graduated and for the first 10 years of my career, I worked as a Clinical Dietitian for Surgical and Medical Weight Loss at the University of Minnesota Medical Center in Minneapolis, MN prior to starting here in Baldwin. I live here in Baldwin with my husband and two daughters.

My role is to ensure that residents are meeting their nutrition needs for health maintenance. Myself as well as the role of the dietary staff, is to ensure that meals are nutritious and tasty and provide the residents with a wide variety to meet their personal expectations. We make sure that residents are provided with choices at each meal and try and make the meal times flexible to allow residents to eat at a time that they prefer. Snacks and beverages are available 24 hours a day in the country kitchens located on each side of the nursing home. Coffee is served daily in the dining areas both morning and afternoon, and we encourage families and friends to join the residents for coffee time, a meal, and special parties.

Angela Tellijohn, Therapeutic Recreation Director

Hello, my name is Angela Tellijohn. I have been the Therapeutic Recreation Director at Baldwin Care Center since April 2017. I have been in my field of Therapeutic Recreation since 2004. I was born and raised in Green Bay, WI where I graduated high school. I attended the University of Wisconsin La Crosse in which I received a Bachelor of Science in Therapeutic Recreation and earned National Certification for Therapeutic Recreation in 2003. I currently live in New Richmond, WI with my wonderful husband (Eric) and son (Grant). I enjoy going to my sons’ sporting events – he plays football and basketball for the NR Tigers, color-art, watching movies, listening to country music, going to concerts, relaxing in my hot tub and playing with our two dogs (Daytona and Chevy).

My responsibilities as the Therapeutic Recreation Director include using a variety of techniques including arts and crafts, pet-therapy, aromatherapy, games, exercise and movement, spiritual wellness, and community outings as a way to improve and maintain physical, mental, and emotional well-being of all the residents at the Baldwin Care Center.

My name is Kevin Brathol and I am the Director of Environmental Services for the Baldwin Care Center, Birchwood Apartments and Birch Haven Assisted Living. I am responsible for the oversight of the housekeeping, laundry, maintenance and transportation staff.

All of the members of my department work very hard to make sure our residents needs are met, and that their stay here is as trouble free and as comfortable as possible.

We do our best to accommodate the wishes of our residents to make their rooms as homelike as possible, bringing in their furniture, pictures, and other belongings.

Sheila Simmons, Office Manager

Hello, my name is Sheila Simmons. I have been employed with the Baldwin Care Center since July 2014. I have worked here as a CNA, Social Service Designee and currently as the Medical Records Secretary.

I have lived in Baldwin, WI for 10 years. I moved here from a small town in southwestern Wisconsin called Shullsburg. In my spare time, I enjoy watching my son play sports such as baseball and basketball, reading, listening to music, going to concerts, and spending time with my family and friends.

I have a wide range of responsibilities at BCC. Some of those include setting up appointments and transportation for the residents, assisting in answering phone calls, census reporting, greeting our guests, and maintaining our medical charts to the proper guidelines. Occasionally, you might also see me working as a CNA.

I really enjoy working at the Baldwin Care Center. I am happy to help in any way possible. Please stop in and say hi anytime.

Hello, my name is Mariah Voeltz and I am the Administrative Assistant for the Baldwin Care Center. I have been employed with the Baldwin Care Center since August 2019 where I started working as a Certified Nursing Assistant and took over as the Administrative Assistant in December 2021.

I am originally from Glenwood City but currently live in Menomonie. I received my Bachelor’s Degree from the University of Wisconsin-Stout in Business Administration with a minor in Investments and Finance in December 2021. Outside of work, I enjoy spending time with my friends and family, attending concerts, creating DIY projects, and cooking.

I have various responsibilities at the Baldwin Care Center including payroll, Insurance, Medicaid and Medicare billing as well as the billing for Birch Haven and the Birchwood Apartments.

I look forward to working with the residents and their families and answer any questions they may have.
